Patient Demographics and Appointments Migration into Osmind

As part of joining Osmind, we can help complete a transfer of your patient demographics and appointments from your legacy EHR.

When a patient is added to Osmind, they will automatically receive an email invitation to join their patient portal. We recommend finalizing your new patient intake flow before we migrate your patients to ensure a smooth onboarding experience.

To successfully migrate data into Osmind, you must use the provided import templates and follow the required formatting rules. Each data type depends on the previous one, so templates must be completed in the correct order.

Template Order (Required)

  1. Providers

  2. Patients

  3. Appointments

Appointments reference both providers and patients, so those templates must be completed before Osmind can import appointment data.

General Migration Rules

Please review these carefully before completing your templates:

  • Do not change column headers in the import templates.

  • All dates must use YYYY-MM-DD format (e.g., 1990-12-28).

  • Leave optional fields blank if you do not have data.

  • Email addresses are used to match existing records. Ensure accuracy and consistency.

  • Remove inactive patients and duplicate records before submitting.

  • Trim extra whitespace from all fields.

  • Save the completed file and submit it to your Osmind contact for review.

Submitting Your Completed Migration Template

Once you have completed your migration template, follow the steps below to securely submit your file to Osmind.

Step 1: Submit the Migration Request Form

Step 2: Attach Your File

When completing the form:

  • Upload your completed migration template in the “Description” field.

  • Ensure the file is finalized before submitting.

  • The file is submitted in .csv or .xlsx format.
